When you make you an appointment, you will be sent a link (via email or a text to your cell phone) for the completion of a few forms. If you are unable to do this electronically, please download and complete of the Patient Intake Form below, and bring it with you on the day of your appointment.
Please arrive at least 15 minutes before your scheduled appointment, whether as a new or established patient.
The below link to the ENT Informational & Educational Guidebook provides a number of tips, remedies and instructions for a variety of common ENT problems. We often give written instruction sheets to our patients during their appointment, many who felt were quite helpful in improving or resolving their particular problem. Feel free to download this booklet.
